CDE 5-Day Advanced Course in Diabetes Care for Health Professionals

 

The state-of-the-art CDE 5-Day Advanced Course in Diabetes Care for Health Professionals is presented regularly at our Centre in Houghton, Johannesburg. The Course is an excellent primer on diabetes and many health care professionals and pharmaceutical industry representatives new to the field have benefited greatly by attending our Courses. Attendance of this Course is a requirement to accredit as a CDE Provider.

  • Application Form (PDF) or Application Form (MS Excel) – The MS Excel version of the form is designed to be completed on a computer (although it can be printed first and filled in neatly by hand) It has help texts associated with certain fields (look for a red triangle in the top right hand corner of applicable fields).

Important Notes:

  2. Please complete the form fully and neatly in block capitals, sign it and fax to +27 (0)86 607-9355. Ensure that you provide us with a valid e-mail address to which we can send your Course confirmation and pre-course readings (N.B. We do NOT fax or courier these).
  3. Places for each Course are limited to a maximum of 30. Early bookings (60-90 Days prior to commencement) are recommended. The first Course in March is likely to be sold out before the end of January 2010.
  4. A place on a Course of choice is subject to:
    5. - Our receipt of a fully completed 2010 application form (One per applicant – you may copy / print forms as needed).
    - The attending delegate is asked to read all these notes and to complete and submit their Application Form PERSONALLY. We will not be liable for any errors, loss or inconvenience resulting from delegation ofzthis task to someone else or from the non-receipt of all the relevant information contained here.
    - Our receipt of full payment (Remember that if you book and pay the full fee at least 30 days prior to a Course commencing, you qualify for a substantial “Early Bird” discount).
    - Availability of places on the Course (places allocated on a “First Come, First Served” Basis).
    - Our e-mail confirmation that we have received your application and allocated you a place on a Course
  5. The Course will run from 08h30 to 17h00 daily. Please ensure that you book your return flights (if applicable) from 18h30 onwards (19h30 onwards for international flights). This will give you enough time to complete the Course and get to the OR Tambo International Airport on time. We would appreciate your assistance in this regard, as it is extremely disruptive to have participants asking to write their final test earlier so that they can catch an early flight. This will not be allowed. If you choose to leave early and miss the final written test, you will forfeit your Official Course completion certificate.
  6. The Wits Health Consortium CPD Office has accredited the Course for 30 CPD points for Medical Practitioners and other Healthcare Practitioners registered with the Health Professions Council of South Africa (Accreditation Number MDB08/271/01/2010). CPD certificates will be provided (if applicable).
  7. Official completion certificates will be provided to delegates who achieve a mark of at least 60% in the final Post-Course Knowledge Evaluation. A distinctive certificate is provided to delegates who achieve a mark of at least 80%
