Benefits for People who have diabetes
The DMP has a fantastic set of benefits and resources to help you manage your diabetes better. The following benefits are available at no further cost to each member (contracted minimum usage applies):
- Consultations with a Doctor specially trained in diabetes management, a Diabetes Educator and a Dietician;
- Annual screening visits to an Ophthalmologist and Podiatrist;
- ALL necessary laboratory tests for diabetes monitoring and management;
- ALL diabetes medication including Insulin and tablets for diabetes;
- ALL diabetes supplies including needles & syringes, blood glucose meters and test strips;
- Access to a 24-hour ”Hot-line” to deal proactively with any acute problems.

Benefits for Doctors
This network is not “exclusive”. Any Doctor who has an interest in managing diabetes but who is frustrated by formularies and other restrictions on their practice; exhausted patient benefits; and poor knowledge of diabetes care can apply to become an Accredited* Preferred Provider for the Centres for Diabetes. You can receive a pre-paid Capitation Fee per month for each patient registered on the Programme at your Centre and thus have the resources to provide full Team Care for your patients
*Accreditation is dependent on:
- Signing of an association contract to provide basket of care according to contractually stipulated “Minimum Care Guidelines”;
- Attendance of:
- An initial specialised CDE training course in diabetes care
- Annual national CDE academic updates; - Willingness to be subject and accountable to Peer review;
- Sufficient interest and knowledge of the management of Diabetes Mellitus to take full risk and responsibility for the costs of any patient hospital admissions for acute diabetes emergencies;
- Successful audit outcomes.
